Questions and answers
The process follows a structured modified 8D analysis, including a structured review of operating history and maintenance records, thermal-hydraulic and system analysis, failure mode assessment, coordination with materials/metallurgical testing as needed, and identification of contributing factors (design, operation, maintenance, or material).
I coordinate with accredited materials testing and metallurgical laboratories or the manufacturer.
I typically request operating data, maintenance and inspection records, P&IDs, component specifications, photos or videos of the failed component, and any prior analysis. A site visit or virtual walkthrough is often helpful but not always required, depending on the nature of the failure.
Most investigations are completed within 2–6 weeks. This includes a delivery of a comprehensive report with a clear executive summary, root cause determination, supporting evidence, and specific, actionable recommendations prioritized by impact and feasibility. I am also also available for follow-up implementation support or client presentations.
